Returning Your Ballot

You must return your absentee ballot to the county election board by mail.

An absentee ballot must be in the hands of the county election board by 7 p.m. on election day to be counted.


Application Forms

Applications for absentee ballots may be made by using the following forms.

Yellow Application for Absentee Ballot Form

Voters may apply for absentee ballots by completing a yellow Application for Absentee Ballots form and mailing it to the county election board office. Voters also may appear personally at the county election board office to complete the form.

Pink Application for Absentee Ballot Form

Nursing home voters, physically incapacitated voters and voters charged with the care of physically incapacitated persons who cannot be left unattended may apply for absentee ballots by completing the pink Application for Absentee Ballots form and mailing it to the county election board office. All pink applications must be transmitted to the county election board office by mail.

Federal Post Card Application

Military and overseas voters may apply for absentee ballots by completing and mailing a Federal Post Card Application, also known as a Department of Defense Form 76. Federal Post Card Applications may be obtained from the voting service officer in a military unit and from United States Embassies and Consulates. All completed Federal Post Card Applications must be transmitted to the county election board office by mail.

Green Application for In-Person Absentee Ballots

Voters may apply for an in-person absentee ballot by completing a green Application for In-Person Absentee Ballots form at the county election board office from 8 a.m. to 6 p.m. on the Friday and Monday before every election.  In federal and state elections only, voters may also vote at the county election board office from 8 a.m. to 1 p.m. on the Saturday before the election.

Application from Emergency Incapacitated Voter

Emergency incapacitated voters may apply for absentee ballots by making a written request. The written request must be signed by the voter or, if the voter is unable to sign, by a witness. The request must be accompanied by a sworn statement from a duly licensed physician. The request must be transmitted to the county election board office by an agent designated by the voter. The white request for Emergency Incapacitated Absentee Ballot and Sworn Statement by the Physician form may be used, but is not required.

Letter Suffices

In all cases, with the exception of the in-person absentee voter, a letter from a voter requesting absentee ballots shall be considered a sufficient application if it contains the information needed to determine the voter's eligibility to vote in the election and to determine where to send the ballots.

Facsimile Application Accepted

Absentee voters, nursing home voters, physically incapacitated voters, voters charged with the care of physically incapacitated persons, military voters and overseas voters may submit applications for absentee ballots to the county election board by electronic facsimile device (a "fax" machine). Voters may transmit one of the absentee ballot application forms or they may transmit a letter.
